- If you use your iPhone/Android as your Dexcom receiver, please call our office and provide an email address we can send an invitation to for sharing.

Glooko (Meters, Omnipod Insulin Pump)
- Please check if your meter and/or insulin pump is compatible with Glooko.
- If your device is compatible, please go to Glooko’s website and either create an account or click “Log In” if you already have a Glooko account. After logging into your account, click “settings>account.” Click “add new code” and then “continue.” Our ProConnect code is wupeds, then click “submit.”

LibreView (FreeStyle Libre CGM only)
- Please log in or create an account and upload your device.
- Please call/email our office or send us a message through MyChart after uploading and provide us your email address so that we can email you an invitation for sharing data.

- Please load Reveal APP on your mobile phone
- Please log in or create an account
- Clinic Code: JD QE TQ
- Select the meter; Reflect, Flex and/or Sync
- Follow App instruction to pair with meter
Tandem T:SLIM X2 Pump with CONTROL-IQ Technology
- Log into t:connect.
- Upload your t:slim X2 pump.
- Please call/email our office or send a message through MyChart after uploading.
- Please provide us with your t:connect log in information (email & password) so that we can review your upload.
